UnityPoint at Home has been recognized by the Centers for Medicare and Medicaid Services (CMS) as a national leader in clinical excellence, the organization announced Thursday, Jan. 29.

The CMS evaluation, which uses comprehensive data from patient assessments, surveys and insurance claims, ranks home health agencies across the country based on quality scorecards. These scores influence future reimbursement and help patients compare care organizations locally and nationally.

CMS determines high-quality home health care based on several key measures, including: patient function at discharge, effectiveness in helping patients remain safely at home, and preventing unnecessary hospital admissions.

All eight UnityPoint at Home agencies across Iowa and Illinois — Cedar Rapids, Central Iowa, Dubuque, Fort Dodge, Sioux City, Storm Lake, Waterloo and Quad Cities — ranked in the top 25% nationally in multiple categories, including improving patient function and preventing hospital stays. Additionally, all agencies ranked in the top 25% nationally for their Home Health Value-Based Purchasing composite scores, which reflects success in helping patients remain in their homes after care.

“This recognition reflects the dedication, compassion and clinical expertise of all our UnityPoint at Home care teams,” said Jenn Ofelt, UnityPoint at Home president. “We’re proud of this recognition because they represent real outcomes that matter to the patients, families and communities we serve.”

UnityPoint at Home is committed to focusing on safety, recovery and helping people live their best lives at home.
